CERTIFICATION OPERATIONS

Applications

The application portfolio is a recording and compilation of documents demonstrating competency in the knowledge and skills specifically related to the functions of a certified.  It consists of:

  • The Application
  • Supervisor Evaluation Form
  • Work Experience Form
  • Colleague Evaluation Forms

The Portfolio Committee meets each quarter to examine files for approval.  The FCB reserves the right to research all experience, documentation and recommendations.  Additional information will be used only to further evaluate an applicantand will be held confidential.
