Mandatory forms are routed to the appropriate individuals for completion on the applicant’s behalf. Once completed, all forms must be collected by the applicant’s designated Point of Contact and submitted to the Florida Certification Board (FCB).

Florida Academy Forms
All forms are built directly into the FCB’s certification system, accessed through the Trainer Portal. All forms are electronic, and sent directly to the specified individual to complete. Completed forms are returned to the Trainer Portal, where they are reviewed by the Point of Contact before submitting to the FCB for processing.
The following forms will be officially retired and removed from the website on September 1, 2026.
